What could cause bleeding from the ears and nose and severe migraine?

My cousin had to go to the hospital because of a severe migraine and he was bleeding from the nose and the ears at one point but the doctor basically accused him of lying. This has been an ongoing problem for at least a week and he went to the hospital before as well. What could cause this?

Bleeding from both the nose and ears is a very uncommon and very disturbing symptom. When the eardrums are intact there is no connection between the outer ear and the outer nostril.

If there was no sign of bleeding through the eardrums then the doctor would have had good reason to be skeptical of the described symptoms.
